目的:探讨分析冷凝集现象对血常规多项参数检测结果的干扰及其消除方法。方法:将4例有冷凝集现象的血常规标本,分别于37℃水浴前、后在Sysmex XN-1000全自动血细胞分析仪上进行全血细胞计数,记录结果并分析细胞分布图。结果:冷凝集现象可导致红细胞计数(RBC)、红细胞比容(Hct)结果假性减低,RBC、Het与血红蛋白浓度(Hb)的关系严重不符,红细胞平均体积(MCV)、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)结果假性增高,白细胞计数(WBC)、血小板计数(PLT)结果减低,但Hb结果无明显变化,并出现直方图、散点图异常,仪器报警提示RBC凝集及双峰RBC分布。标本经37℃水浴20min后,解除冷凝集,然后进行上机检测,此时得到的血常规各项参数检测结果均恢复正常,同时直方图及散点图也恢复正常,没有仪器报警提示。
OBJECTIVE: To investigate the interference of the phenomenon of condensed agglutination on the detection of multiple blood parameters and to eliminate the method. Methods: Four blood samples with condensed agglutination were performed on a Sysmex XN-1000 automatic hematology analyzer before and after water bath at 37 ℃, and the results were recorded and cell distribution was analyzed. Results: Condensation phenomenon can cause red blood cell count (RBC), pseudo-reduction of hematocrit (Hct), severe incompatibility between RBC and Het and hemoglobin (Hb), mean corpuscular volume (MCV), mean erythrocyte hemoglobin MCH). The results of MCHC were falsely increased, WBC and PLT decreased, but the results of Hb did not change obviously. The histogram and scatter plot appeared abnormal, and the instrument alarmed RBC Agglutination and bimodal RBC distribution. After the specimen was immersed in water bath at 37 ℃ for 20 minutes, the coagulation set was released, and then the test was performed on the machine. At this time, the test results of the blood routine parameters returned to normal, and the histogram and scatter chart returned to normal without any alarm.
